To ensure a basic level of understanding of animal welfare responsibilities and occupational health risks associated with animal use, documented receipt and review of Level I Training materials is required for individuals who have not completed Level II Training prior to IACUC approval as a participant. Please answer Yes or No: I have completed Level I Training and verify that I have reviewed the training brochures, entitled: "The Use of Animals in Biomedical Research at the University of Louisville," and "Introduction to Occupational Hazards Associated with the Care and Use of Laboratory Animals.

As Project Director, by submitting this form I understand that the above individual must comply with all IACUC *training and occupational health and safety requirements within 90 days (post-approval). And if the individual is found to be non-compliant after this time, they will be administratively removed from the project(s) indicated and will need to be added again. Finally, I have notified the individual of the possible health risks involved when working with research animals.
