Reinstatement Request Form

Current UofL students who were former members of the University Honors Program and who wish to request to be reinstated in the program once they are eligible (after GPA improvement above the minimum requirement - 3.35 cumulative UofL GPA - or enrollment in Honors classes if suspended for inactivity) may use this form to submit their request.  These requests will be reviewed by Honors, and students will receive decisions via email.  Any questions about the Honors reinstatement process can be directed to honors@louisville.edu or 502-852-6293.
