Faculty and Staff Tuition Waiver Program
The individual below is currently a regular, full-time employee at the University of Louisville. As such, the individual is seeking to participate in the Faculty and Staff Tuition Waiver Program as provided in KRS 164.020(32).
Do not submit prior to May 1 for the Summer Semester.
Do not submit prior to July 1 for the Fall Semester.
Do not submit prior to November 15 for the Spring Semester.
Do not submit until the student is registered for the semester.
Electronic forms cannot be held for later processing.
If this form is disapproved for any reason, the form will need to be resubmitted.
I understand that this information is a reporting requirement of the program. I further understand that I am limited to no more than six credit hours per semester from an educational institution or combination of educational institutions.
