The Dean of Students Office at the University of Louisville continues to partner with faculty, staff, and students to support students for whom any of us have a concern. Over the years, concerns have been shared through phone, e-mail, and face-to-face contact with staff in the Dean of Students Office. These referrals have often enabled University representatives to assist students toward their pursuit of success in and outside of the classroom.

The Student Care Team at the University of Louisville is now offering a tool for faculty, staff, and students to use in sharing concern about a student. Please visit the Guide for Dealing with Students in Distress for assistance in dealing with students.

Faculty, staff, and students are encouraged to share concerns about the well-being of a student with the Dean of Students Office, enabling a more complete profile of the student in distress. This reporting system is in place as one more available tool to report a concern. Phone calls to the Dean of Students Office are still welcomed and encouraged. As always, emergency situations should be immediately directed to the University of Louisville Police Department at 502.852.6111

This is not a system to use for emergencies.

Thank you for submitting a Student Care Team Report a Concern form. Please consider this as confirmation that your report has been received and will be reviewed by a Dean of Students Office (DOS) staff member. A DOS staff member may contact you for additional information. Due to FERPA, DOS may not follow up with you on the results of your report; however, every attempt will be made to contact the student of concern and provide support. If this is an emergency situation, contact the U of L Police Department immediately at (502) 852-6111 or by calling 911. Online Reports are not monitored on weekends, University holidays, or after 5 pm on weekdays.
