SPHIS faculty, staff and students can receive free flu shots on Wednesday, Oct 24, 2007 from 11:30am-1:30pm in room 4007 K Wing. All faculty and staff employed at 80 percent of full time or greater are eligible. UofL students are eligible for a shot as part of their campus health fee.

You must bring your flu shot eligibility form and a valid employee or student ID to receive your shot. Access your eligibility form at http://louisville.edu/campushealth.

Spouses or significant other can receive an immunization but the cost will be $25.
