Please note that students without a Master's degree in statistics, biostatistics, or a related discipline are not eligible to be admitted directly into the doctoral program. Students with an undergraduate degree may apply for the M.S. program. The deadline to apply is March 1 for the following Fall semester (no Spring or Summer admissions). All application materials listed below must be received by March 1.

• - - Transcripts from all colleges and universities attended.
• - - Foreign credential evaluation if applicant did not earn a graduate degree from a US college or university.
• - - At least two letters of recommendation; the standard form is available at http://graduate.louisville.edu/students/grad-rec.pdf or the reference can write a letter and send it directly to the Graduate Admissions Office.
• - - Statement of goals [750 words or less, indicate the program (M.S. or Ph.D.) and emphasis (Biostatistics, Bioinformatics or Decision Science) for which you are applying]; student should indicate in statement if they would like to be considered for an assistantship or fellowship.
• - - GRE scores and percentiles (85th percentile or better on Quantitative section is preferred).
