Laboratory equipment and tools
Library materials

Failure to submit this form and to return all items to the appropriate Speed School offices will result in cancellation of your degree application. If you wish to cancel your degree application for this semester, please check the box and complete personal information only.

Your registration for next semester will be blocked until degree is cancelled or admitted into higher studies. This does not affect your participation in commencement ceremonies.
