This Online Travel Request Form is for Conference Travel Funds funded by the Graduate Student Council and the Graduate School

U of L Graduate Students may apply once per academic year (May 11 - May 10)

Instructions:

First, please read the Administrative Guide for the Disbursal of Research/Travel Awards and the GSC's Travel Reimbursement Procedures at: http://louisville.edu/rso/gsc/travel-reimbursement.html as well as the IMPORTANT NOTES below the form before submitting a request.

Failure to follow the rules and procedures contained therein may result in forfeiture of your funding.

If you do not know the answer to one of the questions please take your best guess or simply enter NA.

• This information is automatically forwarded to Graduate Student Council.
• You will be contacted by the Graduate Student Council.
• The GSC will only assist with Costs for Travel, Registration and Lodging. 
• Save original receipts & submit with a University Travel Expense Voucher (Travel Reimbursement Voucher) upon your return. See http://louisville.edu/rso/gsc/travel-reimbursement.html for more information on how to obtain your reimbursement.
  
• To receive GSC funds,your department must have a current representative serving on GSC.
• Incomplete requests will not be processed.
