Basis for Application
The following is a guideline for the Basis for Application found in Section I of the Affidavit
Student is financially independent person over age 24:
- Letter from employer including dates, position, salary and hours
- Proof of KY state income tax return for the preceding year with W-2
- Proof of current pay stub as evidence of payment withholding tax in Kentucky
- Lease or deed to residence
- Proof of loans/financial aid (if applicable)
- KY driver's license
- KY vehicle registration
- KY voter registration
- Proof of visa status (if applicable)
Student is dependent on an independent spouse OR student and spouse both contribute to household support and are both financially independent of others:
- Spouse's KY driver's license
- Spouse's KY vehicle registration
- Spouse’s KY voter registration
- Spouse's lease or deed to residence
- Proof of payment of KY state income taxes (current pay-stub, tax returns with W2, etc)
- Student's KY driver's license
- Proof of visa status from spouse and applicant (if applicable)
- Letters of employment including dates, position, salary and hours
- If spouse was transferred with employer, please include a letter from his/her employer with details of transfer, position, salary, and hours
- Marriage License
Student is financially independent person under age 24:
- Letter from employer including dates, position, salary and hours
- Proof of KY state income tax return for the preceding year with W-2
- Proof of current pay stub as evidence of payment withholding tax in Kentucky
- KY driver's license
- KY car registration
- KY voter registration
- Lease or deed to residence
- Proof of loans/financial aid (if applicable)
- Notarized letter from parents regarding financial support and previous year’s income tax returns
- Proof of visa status (if applicable)
Student is dependent on parent(s) who live in KY:
- Parent(s) KY driver's license
- Parent(s) KY car registration
- Parent(s) KY voter registration
- Parent(s) lease or deed to residence
- Proof of payment of KY state income taxes (current pay-stub, tax returns, W2, etc)
- Proof of loans/financial aid (if applicable)
- Student's birth certificate
- Proof of visa status from parent(s) and student (if applicable)
Student transferred to KY for employment (or spouse's employment):
- KY driver's license
- KY car registration
- KY voter registration
- Lease or deed to residence
- Proof of payment of KY state income taxes (current pay-stub, tax returns with W2, etc)
- Proof of loans/financial aid (if applicable)
- Letter from employer including details of transfer/start date and salary/hours and location of job
If spouse was transferred with employer, please include a letter from his/her employer including the details and date of the transfer, length of employment with current employer, salary and hours and location of job
- KY driver's license
- KY car registration
- KY voter registration
- Lease or deed to residence
- Proof of payment of KY state income taxes (current pay-stub, tax returns with W2, etc)
- Marriage license
- Proof of visa status (if applicable)
Military member, spouse, or member's dependent child if military member is stationed in KY for active duty service:
- Military orders for active duty service stationed in Kentucky for a period of thirty (30) days or longer (may indicate for an educational purpose)
- Military ID from member, spouse, and/or member's dependent child
Military member, spouse, or member's dependent child if inducted from Kentucky and has maintained KY as Home of Record:
- Enlistment papers as evidence of induction from Kentucky
- Home of Record document
- KY state income tax return with for the preceding year or a current pay stub as evidence of supporting the tax base in Kentucky
- Military ID from member, spouse, and/or member's dependent child
- If being discharged, provide the dischange order. Member must return to KY within six (6) months of discharge
Student is Beneficiary of a KY Educational Savings Plan Trust
- Documentation verifying enrollment in the KY Educational Savings Plan Trust
