Revised Social Behavioral ICF template as of 5/9/2011
As of May 9, 2011, the Biomedical IRB now has a revised consent template on the website. All new studies will be required to have the new template beginning May 16, 2011. The new addition follows local IRB policy:
Compensation - if the subject is to be paid cash or a gift card, it is now required by the University to place a statement about filing a W-9 with the University. Please use the wording exactly as written in the template.
Changes to the consent are to be made either at the next amendment submission or at continuing review.
