Faculty Appointment

Requests for initial faculty appointments shall be made using a triptych. The content of the triptych will be as follows:

LEFT POCKET:

  • P-102 (Biographical Data Sheet)
  • Current Curriculum Vitae (CV)
  • Signed letter of offer (including tenure status)
  • Official transcript for highest degree earned

CENTER POCKET:

  • P-103 (Personnel Recommendation form)
  • Criminal Background Check Consent Form
  • Criminal Background Check Email with Approval
  • Accepted Offer Form from Peoplesoft
  • Job Data Change (JDC) payroll form
  • Position Authorization Request (PAR) form
    (if changes needed to position)
  • Position History Summary
    (including authorization to retain position if replacement)
  • Cover letters from Dean and Chair
  • Copy of Job Description

RIGHT POCKET:

  • Reference letters and/or
    Written documentation of telephone reference checks

The triptych will be forwarded by the Dean to the Center for Faculty Personnel where the material will be reviewed and submitted to Executive Vice President and University Provost for approval. Following approval by the Executive Vice President and University Provost, the Center for Faculty Personnel will place the appointment on the agenda for the next meeting of the Board of Trustees as a recommendation from the President.

After final approval by the Board of Trustees, the Executive Vice President and University Provost will notify the Dean and faculty member of the appointment. The Center for Faculty Personnel will notify University Personnel Services/Payroll of the Board's approval of the appointment. The new faculty member will attend orientation in the Human Resources Department to enroll for payroll and benefits.
