Procards Expiring November 2012

Replacement cards for those expiring at the end of November are now available for pick-up during the following times:

Belknap Office, Houchens LL08K
Tuesdays and Thursdays
9:00am - 12:00pm, 1:00pm - 4:00pm

HSC Office, Kornhauser 101A
Tuesdays
11:00am - 1:00pm

You must bring the following information to pick-up your replacement card:

1. Your driver’s license or university ID card for identification.
2. A completed Certificate of Destruction indicating your existing card has been destroyed,
   OR
   Your existing card that we will be shred in the ProCard Office at the time of pick-up.
3. If you are picking up a card on behalf of a cardholder, you must also bring a completed Proxy Statement.

Any card that is not picked up by Friday, December 7 will be canceled.
