How to Apply
All Applicants:
- Submit the online Upper Division Applicant Data Sheet.
Students are required to satisfactorily complete Pathophysiology NURS 338 and Pharmacology NURS 395 in the summer session prior to fall clinical admissions.

- Submit a Certified Background Check. - Please complete the background check through this link. You will not send this information to the School of Nursing, as it is handled through the outside agency. Please use code NX53 when asked for the package code.
- Please note that all nursing students are required to have background checks. As a result, the Kentucky Board of Nursing requires nursing students to report all misdemeanors and felonies to them. After carefully reading the guidelines on the Kentucky Board of Nursing website, you may contact Associate Dean Dr. Rosalie Mainous at 502-852-8387 should you have any questions.
- Schedule an advising appointment at 502-852-1196 with a Lower Division advisor in the School of Nursing at least 4 weeks before the application deadline to verify all Lower Division course work is complete.
UofL Students Not Enrolled in the School of Nursing Academic Unit:
In addition to the requirements above, applicants not enrolled in the School of Nursing academic unit must also submit:
Transfer Students or Re-Enrolling UofL Students:
In addition to the requirements listed in the first paragraph, transfer students or those who have not remained continuously enrolled at the university, you must also submit:
- University of Louisville Undergraduate Admissions Application. Use NU for both the college and major code. This is the Lower Division code. This will allow the Office of Admissions to review your college credentials for admissions to the university and start the evaluation process of your transfer work.
- Official transcripts from each college or university attended to University of Louisville Office of Admissions. :
