How To Apply
All Applicants:
- Complete the online Upper Division Applicant Data Sheet. This data sheet includes a place to submit a written essay, narrative or short story describing your personal qualities and your short and long term goals regarding your career choice.
- Submit a Background Check. Please complete the background check through this link. The school code is NX 53. You will not
send this information to the School of Nursing, as it is handled
through the outside agency.
Please note that all nursing students are required to have background checks. As a result, the Kentucky Board of Nursing requires nursing students to report all misdemeanors and felonies to them. After carefully reading the guidelines on the Kentucky Board of Nursing website, you may contact Associate Dean Dr. Rosalie Mainous at 502-852-8387 should you have any questions.
UofL Students Not Enrolled in the School of Nursing Academic Unit:
In addition to the requirements above, applicants not enrolled in the School of Nursing academic unit must also submit:
Transfer Students or Re-Enrolling UofL Students:
In addition to the requirements listed in the first section, transfer students or those who have not remained continuously enrolled at the university, you must also submit:
- University of Louisville Undergraduate Admissions Application. Use NU for both the college and major code. This is the Lower Division code. This will allow the Office of Admissions to review your college credentials for admissions to the university and start the evaluation process of your transfer work.
- Official college transcripts to the Office of Admissions. Please mail to:
