Community Music Program (CMP) Registration Form

Registration is available at anytime during the year. All registrations submitted after the beginning of the semester will be pro-rated for the remaining lessons.

Form Instructions: If paying by Credit Card, please complete the form below and click on the Submit button to register.
If paying by check, please print out the form and mail with your check to the address at the bottom of the form.
