David R. Cunningham, Ph.D.

Chief, Division of Communicative Disorders

Dr. Cunningham received his Ph.D. in Audiology in 1972 from the University of Kansas. He held faculty positions at the University of Kansas, University of Missouri and the University of Rochester prior to becoming Director of the Division of Communicative Disorders, Department of Surgery, University of Louisville School of Medicine in 1977. Dr. Cunningham is the founder and co-director of University Audiology Associates, a faculty-owned private practice which supports the Doctor of Audiology (Au.D) Program at the University of Louisville. For nearly twenty years, he was Director of the WHAS Crusade for Children Audiology and Speech Pathology Center at the University of Louisville School of Medicine.

Dr. Cunningham's principle interest is in the area of adult amplification and auditory rehabilitation. His research, teaching and patient care activities focus on these aspects of audiologic practice. Dr. Cunningham co-founded "Hear Care Associates", a dispensing office which serves as a training site for the program's Au.D. students. Dr. Cunningham also teaches coursework and has published in the area of practice management. Dr. Cunningham has over one-hundred publications and specific presentations.

Ian M. Windmill, Ph.D.

Director of Audiology

Dr. Windmill is the Director of Audiology and Assocaite Professor at
the University of Louisville and has clinical interest in the areas of
techniques utilized to assess the auditory and vestibular systems, as
well as the education of audiologists. He is a partner in University
Audiology Associates, a private audiology practice. He is actively
involved in professional affairs through the American Academy of
Audiology and the Accreditation Commission for Audiology Education,
where he serves on the Board of Directors.  He has been actively
involved in numerous local, state and national organizations and
committees and has made more than 100 presentations across the United

Jill Preminger, Ph.D.

Associate Professor of Audiology

Dr. Preminger has been on the faculty in the Program of Audiology since 1998. She teaches courses in the areas of: Acoustics, Speech Acoustics, Psychoacoustics, Gerontology and Pediatric Audiologic Habilitation. Dr Preminger has an active clinical research program which is focused on adult audiologic rehabilitation. This includes speechreading, auditory training and communication training. This also includes aspects of hearing aid fitting, hearing aid use and hearing aid measurement. Dr. Preminger is the President Elect of the Academy of Rehabilitative Audiology, she is an Assistant Editor for the Journal of the American Academy of Audiology and she is on the Education Committee of the American Academy of Audiology. Sue M. Windmill, Au.D.

Director of Audiology, Kosair Children's Hospital

Dr. Sue Windmill received her Masters of Science degrees in both Audiology and Speech Pathology in 1985 and her Doctor of Audiology degree in 2004 from the University of Louisville. She held positions at the Veterans Administration Medical Center and Frazier Rehabilitation Hospital in Louisville, KY before joining University Audiology Associates (UAA) in 1999. Since then, she has dedicated her professional time and experience to pediatrics. She has served as a member of the Board of Directors for the Foundation of the American Academy of Audiology and has served as program chair for the Kentucky Speech and Hearing Association conference.

Dr. Windmill is currently the Director of the Audiology Service at Kosair Children’s Hospital and Director of Pediatric Audiology for University Audiology Associates. Her primary clinical interests are infant assessment and pediatric amplification. Her clinic time is split between Kosair Children’s Hospital and the downtown UAA office. She is also responsible for the internal and external clinical experiences and placements of the University of Louisville doctor of audiology students. (Full CV)

Melanie G. Jones Driscoll, Au.D.

Gratis Faculty

Dr. Driscoll is gratis faculty of Audiology in the Department of Surgery at the University of Louisville School of Medicine. Her research, teaching, and clinical work have focused on auditory processing disorder. Dr. Driscoll is a founding officer of the National Association of Future Doctors of Audiology (NAFDA). Julie M. Manche, Au.D.

Director, Auditory Processing Disorders Program

Dr. Manche is a pediatric audiologist with University Audiology Associates and an assistant clinical professor at the University of Louisville. She received her doctorate degree in 2002 from the University of Florida in Gainesville, FL. She completed her 4 th year externship and one additional year at Christchurch Public Hospital and Hearing Aid and Audiology Clinic in Christchurch, New Zealand. After returning to the United States late in 2003, she joined the faculty and staff at University Audiology Associates.

Dr. Manche is a clinical instructor for the University of Louisville. She gives lectures in the areas of electrophysiology and auditory processing disorders. Her specialties include the evaluation of hearing in infants and children with an emphasis in auditory rehabilitation and auditory processing. She is a member of the American Academy of Audiology and the American Speech-Language-Hearing Association. (Full CV)

Shelly R. Moats, Au.D.

Clinical Audiologist

Dr. Moats is a pediatric Audiologist with University Audiology Associates. She received her Master of Science in Audiology from Purdue University in 1996; and received her Doctor of Audiology from Pennsylvania College of Optometry- School of Audiology in 2006. Prior to joining University Audiology Associates in 2005, Dr. Moats worked in a large private otolaryngology/Audiology clinic in the Minneapolis-St. Paul area.

Currently, Dr. Moats divides her time between Kosair Children's Hospital and University Audiology Associates. Her clinical interests include pediatric amplification and audiologic evaluation for infants and children. Dr. Moats has been actively involved in legislative activities affecting Audiology at the state level, and is currently a member of the Continuing Education subcommittee for the American Academy of Audiology.

Dr. Moats is a clinical instructor in the U of L's Doctor of Audiology (Au.D.) program. She co-teaches the Laboratory Exercises course for first year students, and has provided guest lectures related to diagnostic Audiology. (Full CV)

George O. Purvis, Ph.D.

Chief Audiology and Speech Pathology, V. A.

Dr. Purvis is Chief of the Audiology and Speech Pathology Service at the V. A. Medical Center in Louisville as well as Chief of Audiology at Ireland Army Community Hospital , Ft. Knox , Kentucky and Tricare Audiology Clinic at the Kentucky Air National Guard Base, Louisville , Kentucky . His main clinical interests are in the area of diagnostic assessment of the auditory and vestibular system, hearing conservation and professional issues. Dr. Purvis has served as Chair of both the Kentucky and California Audiology and Speech-Language Pathology licensure boards. He has served on numerous ASHA boards and committees and is currently serving Coordinator of the Audiology and Hearing Science Assembly of the ASHA Legislative Council. (Full CV)

Barbara Eisenmenger , Ph.D.

Clinical Audiologist

Dr. Eisenmenger provides adult clinical audiology services at University Audiology Associates (UAA) and the Ambulatory Care Building (ACB) at University of Louisville Hospital. She also supervises students at those sites. Prior to joining UAA she was on staff at the University of Louisville . She currently holds an adjunct clinical faculty position at U of L and teaches classes in Essential Techniques, Adult Audiologic Rehabilitation, and co-teaches the Cochlear Implant class.

Dr. Eisenmenger began working with single channel cochlear implants when she joined the U of L staff and has been providing adult implant services since that time. She currently works with both Cochlear Americas and Advanced Bionics devices. She is Board Certified in Audiology with a specialty certificate in Cochlear Implants. While at U of L, Dr. Eisenmenger developed and offered audiologic rehabilitation classes to hard-of-hearing patients in the Louisville area.

Dr. Eisenmenger is the professional advisor to the Louisville chapter of Hearing Loss Association of America (formerly SHHH) and she was instrumental in the formation of this group. She also serves as advisor to the recently formed Louisville cochlear implant group. (Full CV)

Fred Wightman, Ph.D.

Heuser Hearing Research Endowed Chair

Fred Wightman

Dr. Wightman is a Professor in the Departments of Psychological and Brain Sciences and Surgery (Audiology). He also holds the Heuser Hearing Research Endowed Chair and is Research Director of the Heuser Hearing Institute. His research interests are in the area of normal and impaired human hearing, with specialization in the areas of spatial hearing/sound localization and auditory attention in children. He has published extensively in the Journal of the Acoustical Society of America, Perception and Psychophysics and other journals and is a past Associate Editor of JASA and a fellow of the Acoustical Society of America. His research has been continuously supported by federal grants (NIH, NSF, NASA, ONR) for the last 25 years. He joined the University of Louisville faculty in July of 2003, after 19 years at the University of Wisconsin. (Full CV)

Richard W. Lazich, Au.D.

Clinical Audiologist

Dr Lazich has over 25 years experience and is a clinical instructor at the University of Louisville School of Medicine. He received his Au.D. from the School of Audiology at the Pennsylvania College of Optometry. Dr. Lazich is a fellow of the American Academy of Audiology and specializes in geriatric hearing loss, hearing aids, and auditory rehabilitation.
