Resident Notes
NOTES FOR NEW RADIOLOGY RESIDENTS
ADDITIONAL NOTES:
Details about ACLS training will come directly from the Department of Anesthesiology, which coordinates this training for house staff. It is best to tell your new residents to obtain this training prior to their arrival here, and they can check with their local hospitals and Red Cross offices for training opportunities. Also, BLS (Basic Life Support) is a prerequisite for this course and must be obtained prior to their ACLS course. Be sure your residents know this. Residents have 30 days from their hire date to provide proof of ACLS certification; those who fail to meet this requirement will be suspended until they do.
Due to time and space limitations, we do not plan to have lab coat fittings as part of House Staff Orientation. Departmental or individual arrangements will need to be made for lab coat fitting, and should be accomplished early enough that residents can receive their coats by July 1st. The name and address of the vendor will be provided by house staff as soon as that information is available.
ADDITIONAL INFORMATION REQUIRED FOR INTERNATIONAL MEDICAL GRADUATES:
- Copy of ECFMG Certificate
- Copy of VISA (for permanent resident or immigrant classification) OR
Copy of IAP - 66 and I - 94 (J - 1 visa holders) OR
J - 1 residents cannot sign up for payroll or start their training until the IAP - 66 has been received and they present the I - 94 stamped "D/S (duration of stay)" - NO EXCEPTIONS.
Copy of naturalization certificate (for naturalized citizens)
Applications for the J - 1 visa will be provided and processed from the Clinical Affairs Office. This process must begin immediately after Match Day, since it takes 3 -4 months to complete. International graduates must present all required immigration documents in order to be admitted to the Payroll Processing portion of Orientation.

- Copy of KY Medical licensure care (for PG - 2's and above)
- Payroll Action Form (PAF)
