       • Chronic obstructive pulmonary disease/ emphysema

       • Interstitial lung disease (pulmonary fibrosis)

       • Sleep-disordered breathing

       • Alveolar hypoventilation disorders

To expedite referral, please provide the following information, if available via fax to 502-852-1359:

  • Recent office notes/Discharge summaries

  • Laboratory data: BC, renal panel, liver panel, uric acid, troponin, BNP, ANA, SCL-70, Rheumatoid factor, HIV and hepatitis serologies

  • Radiology reports (patient should bring actual films to appointment): Chest radiograph, Chest computed tomography (CT), Ventilation/perfusion (V/Q) scan

  • Electrocardiogram (EKG)

  • Echocardiogram

  • Heart catheterization

  • Cardiac stress test

  • Pulmonary function tests

  • Sleep studies and/or overnight oximetry
