Everything you need to know for your first visit to University of Louisville Centers for Primary Care - Downtown and the U of L Health Care Outpatient Center. Call (502) 813-6800 for more information.
Registration
On your first visit to University of Louisville Centers for Primary Care - Downtown, please plan to arrive 20 minutes early. To expedite your registration, please bring the following with you:

A current insurance card.

Current drivers license / photo ID.

Co-pay or deposit (if necessary).

List of your current medicines including the dose and time of day taken.

Pertinent medical records. Please fill out your new patient packet prior to your first visit to Centers for Primary Care - Downtown. Bring these papers to the office to help expedite the check-in process.

U of L accepts most major insurance plans. We recommend that you call
your health insurance company to find out which clinic services are
covered by your individual health plan coverage.
Your Responsibilities:

Make sure you have your current insurance card with you at all times.

Be familiar with your insurance benefits. It is the patient's responsibility to know the insurance carrier's requirements and procedures.

Make sure your physician and the health center are listed as a participating provider in your plan's provider book.
Patient Responsibilities

The patient has the responsibility to be considerate and cooperative in dealing with health care staff and to respect the rights of the fellow patients.

The patient has the responsibility to ask any questions and to seek clarification as may be necessary to adequately understand his or her illness and/or treatment.

The patient has the responsibility to obtain and carefully consider all information he or she may need or desire in order to give informed consent for a procedure and/or treatment.

The patient has the responsibility to weigh the potential consequences of a refusal to comply with instructions or recommendations of the health care provider.

The patient has the responsibility to schedule appointments and to arrive at the office in time for scheduled visits. The patent also has the responsibility to notify us if he or she must cancel or be late for a scheduled appointment.

The patient also has the responsibility to express opinions, concerns or complaints in a constructive manner.

The patient has the responsibility to insure that all information provided for inclusion in his or her record is complete and accurate.
