Palliative Medicine Fellowship Requirements

1. Potential fellows are required to have completed an ACGME approved residency fellowship program (or in 3rd year of residency) in the United States. The residency/fellowship must be in one of the following eleven disciplines: Internal Medicine and its subspecialties, Family Medicine and its subspecialties, Neurology, Physical Medicine and Rehabilitation, Surgery, Pediatrics, Obstetrics/Gynecology, Psychiatry, Anesthesiology, Emergency Medicine, and Radiation Oncology.

2. Passed boards Part 1, 2, and 3 (or in the process of taking Part 3) and must provide copy of board scores.

3. Must be a U.S. citizen or hold a J1 visa. Please provide a current copy of ECFMG certificate, a copy of current visa, or explanation of visa type planned for international graduates.

4. Palliative Medicine Fellowship application on file.

5. One-page personal statement on file.

6. Three letters of recommendation on file.

7. Curriculum vitae on file.

8. Copy of medical school transcripts on file.
