Welcome Current and Future Patients!

There will be no primary care office visit copay at any of our family & geriatric medicine if you and your family's insurance is through University of Louisville or University of Louisville Hospital.   For UofL PCA members, the cost of your office visit will be reduced by $20.  

Appointment Cancellations and Rescheduling

If you have made an appointment at any of our clinics/offices and need to either cancel the appointment or reschedule due to a conflict, please give us 24 hours notice.  This will help us make room for other patients that might need to see our doctors at the last minute.

What to Bring to Your Appointment

1. Insurance Information and Co-Pay
2. Any medications (over the counter & prescription) that you continuously take or are currently taking.  

These two items are extremely important.  Failure to bring us these two pieces of information will result in your appointment being rescheduled at all of our locations.
