Procedures for Bequeathal
Details on how to donate your body to ASNB's Bequeathal Program
- FOUR copies of the bequeathal form and one copy of the vital statistic form can be downloaded and printed from this website. You may also request these forms by mail, e-mail or telephone from Mr. Steve Anderson or Mr. Troy Nukes (see address below). The bequeathal forms should be signed in the presence of two witnesses, particularly those who would be entitled to control the disposition of the donor's remains. Properly completed bequeathal forms have legal status, a formal last will and testament is not required. Relatives of the donor or anyone 18 years of age may be witnesses to this document.

2. For our records, the original bequeahal form along with the original vital statistics form should be forearded to:

3. The remaining three copies should be distributed as follows:
One copy should be kept by the donor
One copy should be kept by the donor's next of kin or a responsible friend
One copy should be given to the donor's physician
