Procedure
for Bequeathal
1. FOUR copies
of the bequeathal form and one copy of the vital statistic form
can be downloaded and printed from this web site. You may also request
these forms by mail, e-mail, or telephone from Mr. Steve Anderson
(see address below). The bequeathal forms should be signed in the
presence of two witnesses, particularly those who would be entitled
to control the disposition of the donor's remains. Properly completed
bequeathal forms have legal status, a formal last will and testament
is not required. Relatives of the donor or anyone eighteen years
of age may be witnesses to this document.

2. For our records,
the original bequeathal form along with the vital statistic form
should be forwarded to:

3. The remaining
three copies of the bequeathal form should be distributed as follows:
one should be kept by the donor, one by the donor's next of kin
or a responsible friend and one should be given to the donor's physician.
This will clarify the donor's wishes to all concerned parties who
will be able to more clearly fulfill the donor's wishes.
