Senior Honors Convocation - Student Information Form

The Prizes and Awards Committee meets in March to select award recipients for the Senior Honors Convocation.  Award selections may be based on academic achievement; however, it is not the only criteria used.  Leadership, service to the community, service to the medical school, and extracurricular activities may be considered as well.

To be considered for an award, you must provide your CV.

If you wish to nominate another student for an award, please complete the Student Information Sheet, making a notation in "Additional Information and/or Comments" that you are nominating another student.
