Urologists are specialists in the field of Urology.  If your doctor or primary care physician has referred you to a urologist, your doctor's office should contact us so we may determine if testing will be required prior to your appointment.

Once the doctor contacts our office, they should fax any records pertaining to your current condition to our office at 502-588-9537.

If the doctor has suggested you contact our office, you will be asked to contact your doctor's office to have records faxed to our office and may be asked to schedule testing if this has yet to be performed.

When calling to set an appointment, you will be asked for information prior to setting the appointment.  Items you will be required to be provide or questions you should expect are:

  • Type of insurance (If Tricare, Passport, or HMO, a referral is required prior to appointment)
  • Demographics (Name, address, phone, social security, insurance ID and group numbers, contact information)
  • Symptoms and/or prior diagnosis
  • Phone number of current physician, medical records pertaining to your current condition, imaging on disc, pathology reports

Within a few days of an appointment being set with our office, you will receive a New Patient Packet in the mail.  If you do not receive the packet prior to your appointment, you may click here to download the packet. If you are unable to complete these documents prior to your scheduled appointment, please arrive at least 15 minutes early.  Patients may be asked to reschedule if paperwork is incomplete.

When you arrive for your appointment, you will be required to provide the following:

Picture ID and current insurance cards

List of current medications and known allergies

X-rays on CD/DVD, lab work, radiology reports (if requested)

As we do not validate parking, please be prepared to pay for metered street or garage parking access.  Norton's parking facilities now accepts credit card, check or cash with current, valid identification.
