Introducing the Kentucky Medical Order for Scope of Treatment (MOST) Form - April 19, 2018

The Kentucky Medical Order for Scope of Treatment (MOST) form is an advance care planning tool signed into legislation in 2015. The MOST form, based on the national POLST Paradigm, offers patients with advanced illness or their surrogates the opportunity for healthcare wishes to be elicited, documented, and honored using this portable medical order. The KY MOST Coalition represents a state-wide stakeholder group promoting the MOST form. The Coalition seeks to transform how KY approaches end-of-life medical decisions by providing greater education and awareness about the MOST form.
