Downloading The Transmittal Form
Please use UARC's transmittal document to create your transmittal for submission with the records you are sending. Follow these steps:
- Download Transmittal.doc. This is a Microsoft Word document. Download (save) the file to the directory of your choice on your PC.
- Review this example transmittal
to see how the form should be completed. (This form requires the free Adobe Acrobat Reader [download].) For instance, all boxes being transferred at one time are usually listed on one transmittal, unless there are distinct groups of records that should be separated. The top section should appear only on the first page and the document is to be completed with additional pages as needed as is normally done in word processing documents. - To use the Word document to create your transmittal: In Word, open Transmittal.Doc from the directory where you saved it.
- We recommend that you use Save As from the File menu to save it under a name reflecting your department (for example, Political Science July 1999 Transmittal.doc).
- Edit the document under its new name to add your transmittal information in the non-shaded areas.
- Save the file under its new name as a Word document.
- E-mail the transmittal file to the University Archives
The next time you need to send a transmittal, use the original Transmittal.doc form again or download another copy from this page.
When the records arrive at the University Archives or the Records Center, they are assigned a unique accession number. This number and the shelf location of the boxes are recorded in the shaded area of the transmittal form and entered into the University Archives and Records Center catalog. We will return a copy to you, usually over e-mail, for your reference in having the accession number and location. If you do not receive a copy with this information within a few weeks, please let us know.
