These forms were created with Microsoft Word, and may be filled out online (for your convenience), printed and turned in to your Supervisor for signature, and submitted to your Faculty Liaison by the 7th of each month. You may also send this completed form to your Supervisor as an attachment. The Supervisor would then electronically sign it and return it to you and then you send it to your Faculty Liaison. It is up to you to get the proper signatures on the forms, and you should discuss this with your Faculty Liaison.
