No Criminal Justice Intern/Practicum agency assumes liability for injury the student might sustain while undergoing the Intern/Practicum.

All Intern/Practicum students will be expected to take out the student health insurance policy, sponserd by the University Student Government, unless they already have an insurance policy affording the same converage.

Intern/Practicum students who are enrolling for summer terms may qualify for the student policy, provided they were enrolled the term prior to Internship. It is the responsiblity of the individual student to work out the details and requirements.
I certify I have read the information above and subscribed to the University of Louisville's Student Government health Insurance or process coverage of another insuracne policy offering equal coverage. I waive the right to submit a claim to, or file action against the Department of Justice Administration, the Intern/Practicum Coordinator, the University of Louisville or any Criminal Justice Internship/Practicum Agency as a result of injuries or illness sustained while participation in the Criminal Justice Intern/Practicum.
Students who furnish false information on this form will be subject to disciplinary action, including possible dismissal from the Criminal Justice Intern/Practicum.
