How to Request New IP Addresses
Or Change Existing IP Addresses
Requests for new IP addresses or changes to an existing IP address are processed through IT Communication Services by submission of a Work Order, formerly the Telephone/Data Service Request Form. All inquiries for changes to or placement of new voice, data or video service must be submitted through this form.
How to request a new IP Address:
Access the work order system and enter the necessary information, including:
- The 12-digit hexadecimal hardware (MAC) address of the new network interface card (NIC).
- The location of the computer, including building and room number.
- The full name of the user of the machine, or responsible person if the machine is in a common area.
How to change an existing IP Address:
Access the work order system and enter the necessary information, including:
- The 12-digit hexadecimal hardware (MAC) address of the existing network interface card (NIC).
- The 12-digit hexadecimal hardware (MAC) address of the new network interface card (NIC).
- The location of the computer, including building and room number.
- The full name of the user of the machine, or responsible person if the machine is in a common area.
