2014 Data Request Form

Please note at this time most ad hoc data requests may require up to 10 business days to complete. Requests that are more complex or comprehensive in nature such as national surveys will require more time. Submission of this form will generate an e-mail to the address you specify below. In approximately 2 business days, you should receive an e-mail from our service account. This e-mail will indicate that your request has been accepted or if additional information and/or clarification is needed. To expedite your request, please include and/or attach relevant instructions and data definitions below.

Data requests that are an institutional priority, including but not limited to those originating from the Office of the President or the Office of the Executive Vice President and Provost, may cause a delay in an already accepted request. In this circumstance, you will be contacted by our office.

Please provide the reason for your request (i.e. accreditation, grant, internal department reporting, creation of publication, external survey, etc.) and the parameters of your request such as the specific semesters, cohorts, majors, degree types, etc. If this request involves a web site, please list the address here.
Upload your survey form, copy of report, or other attachment relating to your request.
