Exit Interview Questionaire
NCAA Bylaw 6.3.2 requires that Division I institutions conduct exit interviews with student-athletes who have completed their eligibility. We are requesting your cooperation by completing this confidential questionnaire and returning it to the respective sports manager. You may also be invited to participate in a personal interview with the Associate Director of Athletics for Compliance and/or Faculty Athletics Representative. Regardless of whether you interview personally or merely return the questionnaire, your identity and responses will remain confidential.
