Exit Surveys
The following surveys were developed to provide you an opportunity to share your experience while employed at UofL and to provide the university with a better understanding of your reasons for leaving.
You may complete either the on-line survey or a paper version. Select the survey that applies to your employment status.

To access the paper version, click on the appropriate PDF, open and print. Complete the survey with a blue or black pen. The survey should be returned to University of Louisville either by fax at 502-852-2344, email at irsurvey@louisville.edu or by regular mail - Office of Institutional Research, 305 MITC, Louisville, KY 40292. Postage will be required if not mailed through campus mail.
