Criminal Background Checks

Under state law, the University of Louisville now requires a Criminal Background Check (CBC) of all new hires. Once a preferred candidate has been selected for a position, a CBC must be initiated.

All CBC requests must use the CBC Agreement Form (PDF). If a new candidate is a minor, please use the CBC Minor Consent Form (PDF).

 
Upon completion, the form must be faxed to Human Resources at (502) 852-5665. The original CBC request should be included with the new hire packet for Payroll.

CBC submissions may take five to seven business days to complete. During this time, all hire requirements to be completed in order to meet the proposed Orientation/start date.

For information on completing CBC requests is available at the CBC Manual for Initial Hires (PDF).  Any candidate who is not currently assigned a Social Security number will still require a CBC. The submission information for these candidates is available at CBC Manual for no SSN (PDF).

In order to ensure privacy for all candidates, departments are requested to use the Criminal History Background Check Fax Cover Sheet (PDF).
