What You Need
Personal Information
For Health, Dental and Vision Plan Enrollment (if you are changing your plan or the people covered)
- Your ULink user ID and password.
- The Social Security Number and birth date of everyone who will be covered by your insurance.
For Get Healthy Now Enrollment
- Required information for the health risk assessment: User name and password (this will be your employee number found on your UofL ID)
Computer System Requirements
You may enroll from a computer at any location at any time during open enrollment.
Equipment
- A computer connected to the Internet (MacIntosh/Apple not recommended)
Internet Browser
- Microsoft Internet Explorer 5.0 or higher
Computers/Vendors Available on Campus
If you do not have access to a computer connected to the Internet (MacIntosh/Apple not recommended) and with Microsoft Internet Explorer 5.0 or higher, computers will be available on these dates:

Human Resources staff will be on hand to help you through the enrollment process and to answer questions.
Benefit vendors on site during open enrollment
Representatives from United Health Care, Anthem Dental, Express Scripts, Chard-Snyder, Anthem Vision, Human Development Company (EAP) and Get Healthy Now will be on site during open enrollment to answer questions:
