UofL offers vision insurance through National Vision Administrators.

What do I have to do during Open Enrollment?

Your responsibilities are:
  • To have vision coverage if you had it in 2011 and to cover the same people: Do nothing.
  • To have vision coverage if you had it in 2011 and to cover different people: Enroll.
  • To have vision coverage in 2012 if you did not have it in 2011: Enroll.
  • If you had vision coverage in 2011 but do not want it in 2012: Once in the Benefits Enrollment portion of ULink, select waive/cancel coverage during open enrollment.

 

Who is eligible for the vision plan?
  • Regular status employees who are employed 80% or greater full-time equivalency (FTE)
  • Regular status part-time employees and temporary part-time lecturers (40% to 79% FTE)
  • UofL retirees and their spouses.
  • Spouses of covered UofL employees
  • Qualifying adult
  • Child(ren) ( natural children, step children, foster children, legally adopted children, and children placed for adoption) to the end of the calendar year in which they turn 26, provided they are not eligible for group health coverage elsewhere.
  • Child(ren) of the employee or the employee’s spouse of any age when such children are incapable of self-support because of a total and permanent disability; and
  • Child(ren) age 26 and under for whom the employee is required to provide health care coverage under a qualified medical child support order (QMCSO), regardless of where the child resides or if the child is dependent upon the employee for support.
  • Surviving spouses of UofL employees or retirees.
  • COBRA participants

ID Cards

You will receive new I.D. cards if you have vision coverage in 2012.

Use: Present to providers in the National Vision Administrators network when you receive services. The in-network provider will submit your claim.

Receipt Date: By January 1, 2012
