Instructions
Your user name and password will be your employee number. You can find it on your UofL ID. After you take the health risk assessment, HealthFitness Corp. will call you to tell you what to do next.
If you would like to register your spouse to participate in Get Healthy Now or you are a person who waives health insurance but would like to participate in the program (campus portion only), please complete the Waiver Registration Form (PDF).
A spouse's user name and password for the health risk assessment is the employee's ID + sp (Example: 1234567sp).
