Our health plan administrator is United HealthCare.  

 

The university will offer the same plans in 2012 that were offered in 2011.

The plans offer variety so you can find one that meets your needs. UofL plans do not require you to name a primary care physician.

Effective August 1, 2011 all University of Louisville health plans give access to a full range of recommended reproductive healthcare services, as preventive services.  These services will have no deductible and no copay if you receive services from an in-network provider with United HealthCare.

YOUR RESPONSIBILITIES:

If you want a voluntary flexible spending account (health care and/or dependent care) you must  make that election. 

If you waive health insurance, and you want your money in a DEPENDENT CARE flexible spending account you must make that election, otherwise we will default your waiver funds into a health care flexible spending account.

If you are happy with the health, dental and/or vision coverage you currently have DO NOTHING.

If you want to change plans or the dependents you cover you must make these changes.

If you want to participate in Get Healthy Now and save $40 month on health insurance you must take the Health Risk Assessment.

Who is eligible for health insurance coverage?

  • Regular status employees who are employed 80% or greater full-time equivalency (FTE)
  • Regular status part-time employees and temporary part-time lecturers (40% to 79% FTE)
  • UofL retirees (under 65) and their spouses (under 65).
  • Spouses of covered UofL employees
  • Qualifying adult
  • Child(ren) ( natural children, step children, foster children, legally adopted children, and children placed for adoption) to the end of the calendar year in which they turn 26, provided they are not eligible for group health coverage elsewhere.
  • Child(ren) of the employee or the employee’s spouse of any age when such children are incapable of self-support because of a total and permanent disability; and
  • Child(ren) age 26 and under for whom the employee is required to provide health care coverage under a qualified medical child support order (QMCSO), regardless of where the child resides or if the child is dependent upon the employee for support.
  • Surviving spouses under age 65 of UofL employees or retirees.
  • COBRA participants

What types of plans are available?

Cardinal Care Plan

This Cardinal Care Plan combines easy access, exceptional clinical care, conveniently located physician offices, low rates and "Cardinal Care" in a network exclusively made up of UofL Physicians http://uoflphysicians.com  The network is limited to UofL Physicians, UofL Hospital and Kosair Hospital for children.  However, it will cover life-or-limb-threatening emergency services or urgent services if you need them while you are outside of the network area. 

Factors to consider in choosing this health plan:

  • The network is limited to UofL Physicians, UofL Hospital and Kosair Hospital for children.  It will cover life-or-limb-threatening emergency services or urgent services if you need them while you are outside of the Cardinal Care network area.
  • Low rates
  • Physicians' offices located at the Health Sciences Center and adjoining Belknap Campus and convenient locations throughout the Louisville area. A cadre of physicians with leading-edge knowledge

Personal Care Account (PCA - HRA)

A personal care account (PCA) is an account with money for you to use for health-care expenditures covered by insurance. UofL contributes $500 for single coverage, $1,000 for employee & spouse and $2,000 for family for the PCA High and Low plans. If your medical expenses typically are under the amount UofL contributes to a PCA, you may benefit from a PCA. The United Healthcare network for a PCA Plan is United Healthcare Choice Plus.

Factors to consider in choosing this health plan:

  • Low monthly contributions
  • If you use all of the money in your PCA, you pay for additional medical expenses out of your pocket until you meet your deductible.
  • You have a higher annual deductible and a higher out-of-pocket maximum.
  • If you don’t use all of the money in your PCA, you carry it over to the next plan year (if you enroll in a PCA). It is possible through roll over of unused funds for you to accumulate up to three times the amount of the annual university PCA contribution.
  • PCA subscribers receive the same prescription drug coverage as the other plans. PCA funds cannot be used to pay for prescriptions.

Preferred Provider Organization (PPO)

A PPO will cover treatment you receive from any doctor or health-care facility, even if they are not in the United Health Care network. If you receive treatment from a doctor or facility that is not in the network, UofL will not reimburse that doctor or health-care facility as much as it would if they were in the UHC network and you will have to pay more. The United Healthcare network for the PPO Plan is United Healthcare Choice Plus.

Factors to consider in choosing this health plan:

  • There is a small deductible
  • UofL pays a percentage of the costs after you meet your deductible.

Exclusive Provider Organization (EPO)

An EPO works similarly to an HMO. Under the EPO, you must receive treatment from doctors or health-care facilities that are in the United Health Care Choice Plus Network, but it will cover life-or-limb-threatening emergency services or urgent services if you need them while you are outside of the UHC network area. The United Healthcare network for the EPO Plan is United Healthcare Choice.

Factors to consider in choosing an EPO:

  • Does not cover out-of-network services except for a life- or limb-threatening emergency.
  • No deductibles.

Physicians or other providers can join or leave the network at any time.

Heads Up

  • If you have a PCA, save receipts every time you use your card. UHC may ask for expense verification if the IRS requires it. UHC may ask for verification even if it processed the original claim.
  • You should consider opening a Flexible Spending Account to cover higher out-of-pocket costs and deductibles with a PCA. (Questions? Call your Benefits Counselor at 852-0040.)
  • If you have a balance left in your 2011 PCA at the end of the year, it will carry over to 2012 if you enroll in a PCA plan for 2012.
