Cardinal Care

The Cardinal Care Plan will be closed to new enrollees. If you are currently enrolled in Cardinal Care for the 2012 plan year, you have the option to remain in the plan for 2013.

The Cardinal Care plan combines easy access, exceptional clinical care, conveniently located physician offices and low rates. The in-network providers consist of UofL Physicians, UofL Hospital and Kosair Hospital for children.  Members will have an out-of-network benefit that consists of providers that are in the United Healthcare Choice network.

The plan pays 60% for out-of-network benefit and the member responsibility will be 40%. In some instances, you may need a service that is not offered by UofL Health Care. In these cases only, you may be referred to a United Healthcare provider and receive in-network Cardinal Care benefits.

 Things to consider in choosing this health plan:

In-network providers are limited to UofL Physicians, UofL Hospital and Kosair Hospital for children. 

Out-of-network consists of providers in the United Healthcare Choice network.
