National Vision Administrators is our vision provider.

National Vision Administrators provides a broad national network. Members may access the provider directory on the National Vision Administrators home page.  The group/sponsor number will be 51756 0000 01. If you need more information on coverage, call the National Vision Administrators Customer Service number at  1-800-672-7723.

Your options include:

• To have vision coverage if you had it in 2012 and to cover the same people: Do nothing.

• To have vision coverage if you had it in 2012 and to cover different people: Enroll.

• To have vision coverage in 2013 if you did not have it in 2012: Enroll.

• If you had vision coverage in 2012 but do not want it in 2013: Once in the Benefits Enrollment portion of ULink, select waive/cancel coverage during open enrollment.
