If you would like to identify a Physician, Hospital, or Urgent Care Center in the Humana network, please visit the Humana Physician Finder Website.
Once there, if you already have your doctor's name and want to know if he/she is in the network, select "Try 'FastFind' --the quickest way to find a provider by name and location!"

Note: Select the Humana ChoiceCare PPO Network from the drop down menu on the “Select Plan” page. The university uses the ChoiceCare PPO network for its EPO, PPO, and Coverage First plans.

Get discounted primary care with UofL physicians!

UofL has worked with the University of Louisville Physicians group to provide discounted primary care to people covered under a UofL health insurance plan. When anyone covered by a UofL EPO or PPO plan visits a primary care UofL faculty physician, there will be no co-payment charge. Anyone with coverage under the PCA Low or PCA High plans will have the cost of the office visit reduced by $20. (Ob/Gyn is considered primary care under UofL plans.)
