Deceased Employee's Family
If you die, and have at least one year of continuous, regular status, 80% or greater FTE service, your spouse and dependent children may remain in the health plan indefinitely.
Continued insurance coverage of your spouse and dependent children would cease at such time as they become eligible for coverage in another group plan or are no longer eligible for coverage under our plan (i.e. children reach limiting age in contract). Your survivor would pay the full cost for this coverage.
