Pre-Dental Hygiene LLC Agreement

The Pre-Dental Hygiene Living-Learning Community brings together students wishing to pursue a career in dental hygiene in a dynamic, engaging environment. This residential community fosters exploration of this career path through courses, programming, services, and connections to campus and community resources. This community will be housed in Community Park Residence Hall.

As a pre-dental hygiene student, you can expect to receive the following services:

a Peer Mentor to assist you with the transition from high school to college
a tutor in your residence hall on some evenings to assist you with homework and studying for tests
opportunities to meet faculty and dental hygienists outside the classroom setting
academic programs to assist you in becoming more successful as a student
social activities to assist you in getting to know community members and the campus better

REQUIRED COURSES:

Fall – DHED 101 (campus culture course) and Chemistry 101

Spring – Biology 102 and 104

(These courses meet General Education requirements whether or not you change majors and whether or not you are currently enrolled in your major.)

Agreement for living in the PDH LLC

As a member of the PDH LLC, I understand the opportunities and the reciprocal commitment outlined below. I understand that it is important for me as a community member to participate.

I will be reassigned (moved to Community Park) in order to participate in this community (Required)

I am currently majoring or intend to major in pre-dental hygiene (Required)

If you already have been assigned, select your current assignment

You will be moved from your current assignment to the Pre-Dental Hygene LLC.

I will register for and actively participate in the courses listed above (Required)

I understand if I choose not to enroll in the core courses I may be asked to move to another floor (Required)

I will keep Thursday evenings beginning September 1, 2012 free for LLC activities, by not registering for classes, discussion sections, or lab sections during this time throughout 2012 – 2013 (Required)

I will participate in DH activities such as the Dental Hygiene Floss Walk and ULSD Smile KY (Required)

I will meet with my assigned mentor if I am a first year student (Required)

Even if you are not a first year student, please initial this field.

I will have the opportunity to participate in observations with a health care provider. Faculty and staff in the program will assist me in exploring opportunities (Required)

I will abide by all relevant Housing and Residence Life regulations (Required)

I will commit myself to learning about myself and others as I engage in the LLC (Required)

I will actively participate in community activities each semester (Required)

I will attend the required LLC activities: (Required)
