Student Teaching Application Fall 2012

All student teaching placements are made through the Office of Educator Development and Clinical Practice (OEDCP). University students should NOT contact the district/school directly for student teaching placements.

Step 1- Student Teaching Placement Form
Due: February 24, 2012

The Student Teaching Placement Form provides detailed information necessary for the OEDCP to make the best placement for candidates. The estimated time to complete the Student Teaching Placement Form is 10 to 15 minutes.

Step 2- Medical Examination and TB Test Results
Due: April 13, 2012

State regulation requires a current medical exam and TB test. The Education Professional Standards Board (EPSB) has defined "current" as within one (1) year prior to student teaching. A physician may not waive the requirement for the TB test by indicating "not at risk." The OEDCP must have a current TB test on file.

The University of Louisville Medical Examination Form for Admittance into Student Teaching is offered as the preferred document; however, other standard medical examination forms will be accepted.

Candidates having difficulty scheduling a medical exam/TB test by the deadline should notify the placement office: danna.morrison@louisville.edu; maxine.elliott@louisville.edu or dsoake01@louisville.edu

Step 3- Criminal Records Check
Due: April 16-May 11, 2012

Criminal Records Checks cannot be completed until placements are confirmed. Criminal Records Checks are completed at the individual district Board of Education/Central Office. The fee for a Criminal Records Check is $35-$40 depending on the district. Once placements are confirmed, candidates will be provided additional information regarding the process, fee and district contact person for the district where they will be student teaching.

The following verification form should be completed and returned to the OEDCP Office 285 A or B or scanned to dsoake01@louisville.edu or danna.morrison@louisville.edu

Step 4- Student Teaching Handbook Verification
Due: April 13, 2012

The Student Teaching Handbook serves as a guide to program policies and procedures, participant responsibilities, and an overview of the initial process involved in the certification and subsequent readiness for employment as a professional educator. Student teacher candidates must verify that the handbook has been read.

Student Information Permission Form
Due: April 12, 2012

This Student Information Permission Form gives the OEDCP permission to provide districts who inquire with contact information for the purposes of recruitment.
