Procedure for Addressing Problems Occurring During Student Teaching

Issues and concerns related to candidate performance in student teaching may arise. Whenever the cooperating teacher, the university supervisor or the student teacher has a concern, he/she should complete a Communication of Concern form.

On this form, the writer describes the concern and establishes a date to discuss the concern among the three parties.

Subsequently, the three parties create, in writing, a plan of action for addressing the concern, including the support that will be provided by each appropriate party. A date by which the plan of action is to be completed is also established.

Possible consequences may also be specified, in case the action plan does not lead to the resolution of the concern.
