To sign-up for the Fitness Evaluation Program, follow the steps below to get your fitness evaluation:
- Print out this page and fill out the form below
- Mail the form and your payment to the address below. Make checks and money orders payable to UofL Exercise Physiology Lab:

Louisville, KY 40292 - We will send you all the necessary screening forms to complete include a waiver form, medical history form, and physician clearance form.
- Complete all forms and return in the envelope provided.
- Our staff will verify physician clearance and determine if you can participate in this program.
- If you are eligible to participate in this program we will call you and schedule your appointment. If you are not eligible to participate we will return your check or money order.
