1. Participant must attend one or more institute session(s)

   Attendance will be determined using GoToWebinar's Attendance Report feature. Each participant's total time will be added together to determine the number of hours earned. Since this feature only keeps track of individually registered participants, it is imperative that each participant log themselves in to the webinar using a separate machine. Multiple individuals using the same machine at the same time will result in only the individual who actually logged in to the webinar being eligible for earning credit for attendance with the exception noted below.

   **NOTE: If it is not possible for you to log into GoToWebinar individually (i.e. if you can only access the conference by sharing a computer with another attendee), you must contact this office at 502-852-3274 or b.kinney@louisville.edu prior to June 18th. Failure to do so will result in not receiving PD/EILA credit for any sessions attended.

2. Participant must complete a "Session Evaluation and Feedback Form" for each session that PD/EILA credit is desired

   Once attendance for a session has been verified, evidence of a completed feedback form will also be verified (the form can be found at http://louisville.edu/ciidl/summer08/feedback).

   If the paticipant attends a session but does not complete a feedback form for the session, PD/EILA credit for that session can not be issued. Likewise, if a feedback form is submitted, but there is no evidence of attendance, PD/EILA credit for that session can not be issued.

   PD/EILA certificates will be mailed to the address provided on the feedback form within two weeks of verifying both attendance and receipt of feedback form(s).

Note: PD/EILA credit WILL NOT be available for viewing pre-recorded sessions.
