Final Note Taking Evaluation
Thank you again for your service! It's that time of year for you to give us a final evaluation of your note taking experience this semester. Not only does your response assist us in providing and maintaining quality accommodation services, but it also gives us an indication of your student’s participation throughout the semester. If you would like a certificate indicating your service hours, please contact Valeria Aramburu at v0aram01@louisville.edu. If you took notes in multiple classes or for multiple students, please fill out a separate evaluation for each student in each class.
