Final Note Taker Evaluation

Hello, everyone! It's that time of year again for you to give us an evaluation of your note-taker(s). Not only does your response assist us in seeking and maintaining quality note-takers, but it also gives us an indication of how good your note-taker’s performance has been throughout the semester.
