Program of Study Form
To be completed once a complete program of study has been agreed between the student and advisor(s)
Thesis Advisory Committee Form
This form creates the Thesis Committee for the student, and requires the Program Director's approval. The form should be completed before the middle of the student's second semester.
Annual Progress Report
This form assesses student progress and outlines both goals met and academic targets to be achieved. This form should be completed every time the student and thesis committee meet - minimally once per year.
